Non-surgical rhinoplasty

In some patients correction of the nose shape can be achieved by non-surgical means. Instead of traditional nose job, we can inject a filler, such as Radiesse. BOTAX - new plastic surgery tax

I am writing you today about an issue that affects everyone who is receiving or is considering aesthetic medical treatments from a licensed and specialized physician, including botulinum toxin and dermal filler injections, microdermabrasion, dental caps and implants, teeth whitening, laser hair removal, and many more.

To help pay for the health care reform bill that is being debated over the next few weeks by the U.S. Senate, users of such cosmetic medical treatments like yourself have been unfairly targeted with a 5% federal tax on these procedures. As your physician specialized in aesthetic medical treatment for 15 years, I oppose this tax for several reasons outlined below:

· The tax is punitive and places an additional burden on the middle class – According to the American Society of Plastic Surgeons, sixty percent of patients considering a cosmetic medical procedure are squarely in the middle class, earning between $30,000 and $90,000 per year. This tax would come on top of increased payroll and income taxes sure to result from health reform’s $849 billion cost, and would penalize those who have made a well-educated and informed decision in consultation with a specialized physician to opt for cosmetic medical treatments because they allow them to express their best self and have been scientifically tested.

· The tax discriminates predominantly against women – Medical society data shows that 91 percent of patients seeking cosmetic medical procedures are women. And many of these women are seeking treatment to improve their appearance to enhance their self-image and self-confidence which helps them succeed at work and in other areas of their lives.

· The tax does not serve the goals of health reform – Since these procedures are generally not covered by insurance, the proposed tax will not reduce the cost of health care nor will it reduce insurance premiums. Further, it will increase administrative costs for physician offices.

· The tax puts cosmetic procedures in the same category as cigarettes and alcohol – So called “sin taxes” on cigarettes and alcohol were imposed to try to change unhealthy behavior and cover the significant costs that these products placed on the health care system. This is logical. But a tax on cosmetic medical procedures is illogical because they are healthy and merely a responsible form of self-improvement. Furthermore, these procedures do not impact the heath care system since patients pay for these procedures themselves and they are not covered by insurance. People should have the freedom to decide what is best for their personal health care, including our own aesthetic appearance and therefore be free to spend our own, hard-earned money to correct, alter or enhance our appearance responsibly, in consultation with a physician, without being forced to pay an extra tax

If you agree talk to your legislator. Obviously, tax on lawyers services and tort reform are not considered as a way to pay for and improve health care

The name of the face game -FAT

It is interesting how facial rejuvenation changed over the last few years! Instead of just removing fat I prefer now to TRANSFER it to a different location. Fat transfer allows for more natural and youthful look rather than skeletonized look of previous cosmetic facial surgery. Like in this case where fat was removed from the baggy eyes and put into cheeks

Better survival for breast reconstruction patients!

Interesting news: Immediate reconstruction following mastectomy is associated with a 26% reduction in breast cancer-specific mortality, according to a report in the October 15th issue of Cancer

As noted, women who had any type of immediate breast reconstruction, including implant and autologous reconstruction, had a 26% decrease in breast cancer-specific mortality compared with women who underwent mastectomy alone, the authors report.

The improved survival was present among women in all age groups, with those 70 years of age and older having the lowest risk of death among all those undergoing reconstruction.

We have seen this in our practice and now it does seem to be real!

But if one cannot come with more than a few good results - it is bad. LifeStyle lift was penalized in July and agreed to pay $ 300 000 to settle the claim and promised to refrain from publishing any further fake positive reviews on their website and Internet message boards and other advertisements. Apparently , LifeStyle lift used their employees who never had that procedure to post enthusiastic reviews and instructed them to "write frequently about the great experience they had"

American Society of Plastic Surgeons published 2008 statistic on plastic surgery procedures. Here it is:

This list pretty much follows what I observe in my practice. Non-invasive treatments are on the rise because they are safer and more predictable. At the same time surgeries like breast enhancement went up 64% over last six years. Nose job remains to be very popular procedure although as compared to breast implants this number is split between man and women :-)

AAAHC survey

Our office based surgery center underwent survey ( required every 3 years) by the AAAHC. The Accreditation Association for Ambulatory Health Care - also known as AAAHC or the Accreditation Association - is a private, non-profit organization formed in 1979 to assist ambulatory health care organizations improve the quality of care provided to patients. We have been accredited by the AAAHC for over 3 years and it seems that we passed again with all As!

Dr Turowski passed re-certification Board exam !

Dr Gregory Turowski passed the required re-certification exam of the American Board of Plastic Surgery with a 99% score! To meet the the highest standards of plastic surgery the re-certification exam is required by the Board every 10 years. To be admitted for the examination the doctor must meet strict criteria. Surgeon must be Board Certified by the American Board of Plastic Surgery. The Board reviews and approves doctor's cases and results and checks the ethical standards. All cases must be done in the accredited facilities.

Evolence FDA approved for 12 months

Filler Evolence got approved by the FDA as lasting 12 months! I always liked it. It causes less bruises and swelling than Radiesse or Restylane. However it gives less volume per syringe, cost is an issue. However, for those that rather spend more money and have no swelling/bruising it is worth it.

New Botox - is it time?

Medicis corporation announced approval of Dysport. This is a new product directly competing with Botox. As a plastic surgeon I sincerely welcome addition of this new product. For years Botox was the only choice for temporary active wrinkle reduction. Healthy competition s always a good thing! However, I am not sure how soon I will feel comfortable recommending this new product to my patients. There is just not enough USA experience. Stay tuned! In the meantime this should help our patients by lowering prices of Botox! 

Breast augmentation - still number ONE!

Breast augmentation remains one of the most popular procedures performed in our office (as is true across the USA according to the official statistics). Some women, like current Miss California, can count on their employers to cover the costs of surgery. Fractional CO 2 Laser skin resurfacing

In the continuing quest of facial rejuvenation, our practice is now offering the fractional CO2 laser technology. Laser skin resurfacing with the traditional CO2 laser was and still is the gold standard for skin tightening and improvement in pigmentation and texture. Unfortunately, this older treatment resulted in many side effects and complications. Today we offer this same wavelength of light, CO2 modified in a fashion which gives us similar improvements in the skin with many less complications and less down time for our patients. Dr Turowski is pleased to congratulate his former professors and colleagues at the Plastic Surgery Division of  the Harvard Brigham and Women's Hospital in Boston on the breakthrough operation of the 2nd face transplant in the USA. They continue tradition at Brigham, where former chairman of Plastic Surgery performed first kidney transplant in the world  (and became a Nobel prize laureate)

American - Brazilian Plastic Surgery Meeting

Dr Turowski has just returned from the American Brazilian plastic surgery meeting in Park City Utah. This was a great occasion to exchange experiences between American plastic surgeons and our Brazilian friends.  General consensus of the meeting was that  less invasive procedures are becoming more popular. Non-invasive treatments are booming (Botox, injectables, lasers). There is more and more interest in laser liposuction/skin tightening and mini face lifts. Axillary silicone breast augmentations are very popular in Brazil - scars are not visible during topless  Rio Carnival! (I agree scars less visible than in other locations and we should use this approach more often). Als fat injection is here to stay. Both for the "Brazilian butt lift" and for the facial rejuvenation

Beware of counterfeit injectable products

Many physicians have alerted authorities to the dangers of cheaper, unbranded, unapproved cosmetic injectables. Disreputable injectors and counterfeit, imported injectables present the greatest risk to a patient's appearance , and ultimately one's health.  There are professionals who inject substitute botulinum toxin instead of Botox. There are already 68 arrests and 29 convictions of health professionals. Thera are fillers such as liquid silicone that may cause serious health problems.

Economic crisis

Interesting news from the plastic surgery society and media polls: despite bad news about the economy the numbers for the non-invasive procedures are growing and numbers of cosmetic surgery procedures is not declining. Breast augmentation incision/scar

Interesting news about breast augmentation: Recently published study (Plastic Surgery Journal) confirmed what Dr Turowski has been observing over the years. He always thought that periareolar (below the nipple) incision is the preferable spot for breast implant insertion for breast enhancement with the silicone breasts implants. Not only it is less noticable and more predictable in apearance than inframmary (under the breast) scar but also causes less numbness/sensation loss. Recent study from Vanderbilt University confirmed that under the breast incision caused more numbness and nipple sensation loss than periarealorar incision. Axillary (armpit) incision remains his preferable spot for saline breast augmentation

Revision rhinoplasty

Although nose job (rhinoplasty) can usually be performed in quite predictable way, sometimes the results are not what one originally expected. Dr Turowski is very experienced in nose reshaping surgery and, therefore, frequently patients turn to him to correct previously less than satisfactory results. This is a recent example of the patient who underwent a rhinoplasty by another plastic surgeon. An open rhinoplasty was performed to correct this less than ideal result.
